Greater Manchester Pre-operative Smoking Cessation Audit for Orthopaedics

Abstract Background: There is evidence of the benefits of pre-operative smoking cessation prior to surgery. We aimed to audit smoking cessation services for pre-operative orthopaedic surgery patients within Greater Manchester.
Methodology: 9 acute trusts participated. Pre-op staff were asked to identify 40 current smokers listed for orthopaedic surgery. A follow-up questionnaire was sent to each patient post-surgery to corroborate the promotion and uptake of stop smoking services.
Results: 314 patients had their case notes audited and 24% responded to the survey. Overall trusts did not meet the recommended 100% standards for: discussing the surgical benefits of quitting (evidenced in 27% of casenotes and 57% from survey); establishing motivation to quit (evidenced in 43% of casenotes and 68% from survey); and offering a referral (evidenced in 62% of case notes and 30% from survey). Referral uptake was 11% (n=34), yet 31% of survey responders reported quitting and 38% reported reducing the amount they smoke prior to surgery.
Conclusions: The evidence suggests that more can be done to support smokers to quit prior to surgery. It is recommended that trusts re-evaluate their assessment/referral pathways to ensure patients are fully informed of the benefits of quitting prior to surgery and their options for further support.

Presenter biography Biographies:
Deborah Lee: Deborah’s academic background lies in psychology and she has eight years’ experience working within the NHS and 6 years within a public health role. Deborah has coordinated and led on research, audit and service development projects within the NHS, has practical experience in qualitative and quantitative research methods, and has experience writing papers for publication. The main focus of her work centres on health promotion and service development within acute settings. Her role in this project was to lead, coordinate and manage the supra-district audit across multiple sites, ensuring accurate data collection and overseeing the evaluation and interpretation of findings.
Bethany Brown: Following the completion of her First Class BSc (Hons) in Psychology with Counselling and Psychotherapy Studies, Bethany began working in the NHS. Drawing on her research experience of working with vulnerable people, she completed patient experience and equality and diversity audits in an acute hospital setting. In her public health-focussed position at Stockport NHS FT, Bethany has contributed to clinical audits and service evaluations. Her role in the current project was to provide statistical analysis and a comprehensive feedback report, both at supra-district and individual trust level.
