Integration of healthy living services

Abstract n September 2012 we began to deliver a healthy lifestyle service, this included a team that has the ability to deliver multiple interventions including smoking cessation, weight management and alcohol reduction. The service is relatively small and is only accessible through being referred by GP Practice staff following an NHS health check. This staged approach would ensure that we could manage the potential volume of customers wishing to access this type of service, but also target specific areas of greatest need.
The three topic areas already had well established core specialist services that supported the new concept of service delivery. The core services provided training, mentorship and helped develop the protocols and procedures that would underpin service delivery. The development of care pathways and service criteria in collaboration with the core services were fundamental to providing the best possible care to the service user.
Strong evidence base backed up by clear outcomes were established within the service. This would enable us to demonstrate value for money and align the outcomes to citywide priorities.
Reflecting on the process so far, we have had over 500 referrals and have clear evidence that this type of service can be successful, with a number of success criteria identified.

Presenter biography Karen Haw is the Head of Leeds NHS Stop Smoking Service and Healthy Lifestyle Service
Karen has lived and worked in Leeds most of her life, she qualified as a General nurse in 1985. After a short period working within a hospital setting, she pursued a career in community nursing, where she gained experience of working within the field of smoking cessation. In 2000 she moved full time into the field of smoking cessation first as an advisor then managing the service in 2007.
More recently although still passionate about smoking cessation herself and a group of others developed a more integrated approach to health improvement. The healthy lifestyle service enables clients to work on multiply lifestyle changes including smoking cessation, weight management and alcohol reduction.
