Abstract August 2011 saw the launch of the Secondary Care Stop Smoking Service in County Durham & Darlington Foundation Trust. The Secondary Care team consisted of 4 Specialist Stop Smoking Advisors and a full time Administrator across two acute sites, University Hospital North Durham and Darlington Memorial Hospital. The aim of the project was to target the priority wards of Respiratory, Cardiology & Surgery, Advisors were targeted to engage ward staff and to raise awareness of the service and promote referral to the Stop Smoking Service. To promote the service to across all wards, Advisors provide Very Brief Advice training on RGN Mandatory Training days and have produce 2 supporting e-learning programmes, in addition a full Stop Smoking Service is available to all staff within the hospital. Support and NRT was made available for patients both making Quit attempts but also Temporary Abstinence. A discharge pathway was established that enabled all patients referred to be offered a follow up appointment in the community with County Durham & Darlington Stop Smoking Service.
18 months on a review was conducted to see what had been achieved, what the challenges were and the plans for the future.

Presenter biography Barbara Broadfoot has been a Specialist Stop Smoking advisor with Co Durham & Darlington for 11 years. Barbara has a nursing background and was a Specialist Stop Smoking Advisor in Primary Care for many years and took the opportunity to work with the Secondary Care team in 2011.
Claire Askew has been a Specialist Stop Smoking advisor with Co Durham & Darlington for 8 years. Claire has a nursing background and was a Specialist Stop Smoking Advisor working across a variety of settings in the community for many years before joining the Secondary Care team in 2011.
