Epidemiology of smoking in latin american middle-aged and older adults: the Platino study

Abstract
Background:
Smoking is a leading preventable cause of death and disability worldwide. Standardized data on the prevalence of smoking is lacking in Latin America. The aim of this paper was to evaluate the epidemiology of smoking in five Latin American sites.
Methods: Multicenter study including population-based surveys in five sites: Sao Paulo (Brazil; N=1,000), Santiago (Chile; N=1,208), Mexico City (Mexico; N=1,063), Montevideo (Uruguay; N=943) and Caracas (Venezuela; N=1,357). Individuals aged 40 years or more were eligible. The outcome variables were smoking status (never, former or current), age at beginning and stopping, and pack-years of smoking.
Results: The prevalence of smoking ranged from 23.9% (95%CI 21.3;26.6) in Sao Paulo to 38.5% (95%CI 35.7;41.2) in Santiago. Males were more likely to smoke than females in all sites. In most sites and in both genders, the peak prevalence was observed between 20-29 years, while the peak incidence was observed between 10-19 years. Age at starting smoking was not clearly associated with birth decade among males, while among females, it tended to decline over time. Age was inversely associated with smoking in all sites and in both genders. After adjustment for age, no consistent effect of schooling on the prevalence of smoking was detected.
Conclusion:
The prevalence of smoking varied considerably across the sites; Chile presented the highest prevalence. A consistent reduction in the mean age of smoking onset over time was observed among females, but not among males, highlighting the need for gender-specific approaches to prevention.
