Effectiveness of an intervention on smoking among school-adged adolescents in Brazil

Abstract
The Brazilian Ministry of Health affiliated with the National Institute of Cancer (INCA) is proposing a school-based intervention on smoking to be applied in the whole country. In order to test the effectiveness of the intervention, a study was carried out in Pelotas, southern Brazil. Thirty two schools were sampled, and randomly assigned to receive the intervention or not. Students of the 7th and 8th grades of the schools sampled answered a baseline questionnaire on smoking habits and knowledge. Prevalence of smoking (at least one cigarette in the previous 30 days) and knowledge of smoking were similar across the groups at baseline. INCA professionals trained school teachers and supervisors on the intervention, which consisted of including smoking-related subjects at classes, folders, educative materials. In the middle of the year, three INCA professional visited each school of the intervention group to measure the dose of the intervention. In the end of the year, the questionnaire was applied to all students of both groups. Prevalence of smoking in the post-intervention phase was 7.9% in the control and 7.1% in the intervention group. The P value for the interaction between group (control vs. intervention) and time (baseline vs. post-intervention) was 0.48, indicating no effect of the intervention on smoking habits. Smoking knowledge increased markedly among students from the intervention group; this was not observed among the controls. In conclusion, a one-year intervention on smoking among school-aged adolescents was effective to increase knowledge on the harmful effects of tobacco use, but could not produce positive effects on behavior.
